medical info needed from SAW III movie

    ha, I want to clarify that I haven't allowed my children to watch ANY of the Saw movies either.

    My youngest plays one rated "T" videogame and that's some Spiderman game.

    I thought they were rated "T" more due to the difficulty of the game rather than the...more adult content?
    I guess it's silly of me to think that. D'oh!

    I haven't seen this movie (and wouldn't unless it was under duress!) I think I would say something along the lines of "Well, that's how Hollywood does it." Very little of what Hollywood does, even the gross stuff, resembles real life.

    The thing is.. that what they did to save this guys life in the movie by relieving pressure as using a saw to remove part of the skull ACTUALLY IS what neurosurgeons do with our bodies. So it is realistic in that respect.[:)]

    I'm no medical expert, but it seemed like the procedure itself wasn't that far fetched, however she kept Jiggsaw awake, if I remember correctly, because she didn't have the right equipment to anaesthetize him and monitor his vitals.  I can't remember what she gave him for pain, but basically she made a few incisions with the scalpal, peeled back the scalp, drilled four holes (with a cap on the bit that prevented the drill from going too far in) and then cut between the corners with a Stryker saw.  That part of the movie was actually far less gruesome and far fetched than most of what I watch on TLC's "Untold Stories of the E.R" (true stuff).

    I've actually had a few patients with a "bone hawk" where we did bilateral craniectomies to relieve pressure.  When they improve, the skin sinks in on both sides and they look creepy with a capital C!  We put the pieces back on eventually or put prosthetic pieces (if they recover).  Craniectomies (opening the skull and taking the piece of skull off-not replacing it) are actually quite contraversial.  The only data on them shows that these patients have improved survival but not improved neurologic outcome.  Many centers won't do it, they just do a craniotomy, evac the hematoma and place an EVD (external ventricular drain).  Our neurosurgeons are fond of the craniectomy for whatever reason.  Not that this has anything with what you are asking about shelly, sorry I've been up for 36 hours :)

    This is a little off-topic, but maybe Ottoluv can answer for me - I remember reading that Roald Dahl had invented some sort of shunt that relieves pressure and fluid in the heads of children, is that true and is his invention still used?  I think he's a pretty crazy/cool guy regardless.

    exactly what I have heard.... their lives are often saved, but they don't have the best recovery possible.

    oh gosh.. PLEASE no worries.. in my line of work it is actually very fascinating to discuss this.   I love when I get to train neurosurgeons how to teach injury prevention to kids.. I get to sit down and pick their brain (if you will) on surgery techniques.. I am kinda a freak I guess![;)]

    I love him as well, My favorite was that short story where the lady stuffs her house guests, freaky!!  From what I remember, he didn't invent the VP (ventriculo-peritoneal) shunt, his son Theo had problems with the valve clotting when the pressure was normal or low in his cranium.  He developed some different type of valve that still worked with normal pressure or negative pressure hydrocephalus.  I'll call my buddy who is a neurosurg resident and see if he remembers anything else :)

    I was there last weekend, I'm in west hollywood now (that's where I live). I went to UCSD and all my family lives off the 8 in san diego by SDSU and in del mar :) I hope to move back down when I graduate from residency. I was shmoozing with the chairman from the Scripps vascular fellowship ha ha. Sorry to hijack!
