I agree he is just trying to appease you - please don't get mad at him because you assume "he knows he did wrong" - this weill confuse and upset him needlessly and won't help with the problem. If you find any mess, put him away somewhere quietly while you clean up using a biological cleaner. It won't do any good "showing it to him" - if he does make the connection he is just likely to get neurotic about toileting which will make the problem far worse.
As well as a vet check, go back to basics with house training - lots of supervision, confinement when you can't watch him and yummy treats and "parties" when he does it outside. If the vet rules out a UTI, then I would lay money on him being back on track before long.
Don't wait for him to "tell" you he needs to go out - keep him on a schedule. He is less likely to have accidents that way.
