Hmmm, if it's this extreme and medical causes are ruled out, I'd do it the same way we do with dogs - start smaller. We had a cat with a similar problem (she was feral....litterbox, what's that?). What we ended up doing was essentially crating the cat with a litterbox, but instead of using a crate, we built a two-level enclosure out of these wire cubes you can get at Target. We use plastic cable ties and made a big kitty cage. This encouraged her to start using the box. Then you can open it up and give the cat access to the room, more and more until she is consistent with the box. Unfortunately our cat died of complications with FIP so I can't say whether it worked but this is what the vet suggested after we tried confining her (would just fine another spot to pee) and various types of litter.
