I would probably start looking towards Hill's Z/D or Purina's HA. They are each company's version of a hypoallergenic food.
That's an interesting suggestion. I know Kaiser has anal gland issues; Jessie, who is mostly GSD, used to need them expressed every four to six weeks. I changed her to a RX diet last summer ( a fish and rice diet by Royal Canin) because of her food and environmental allergies, and she doesn't need them expressed anymore. I don't know if it's because she isn't allergic to the food or because it has anti inflammatory ingredients; high in omega 3's, has curcumin, aloe vera, extra zinc and b vitamins for the skin. A simple diet high in omega 3's may help Kaiser. I'm sorry to hear that the fistula is so big; poor Kaiser.
