I think this diet has waaaaay too much chicken and not enough red meat.Not knowing whats in your vitamin, it looks like you have about half the potassium you need,half the zinc, copper is very low, manganese very low, half the magnesium, half the iron.(This is just a quickie look).

Feeding the fish once a week does't make sense to me, your dog needs omega threes every day, not just once a week? but I'm no expert.
