The head tilt can become habit -- You might try massage on the neck to help that. That helped Foxy's a great deal.
The thing that vets always worry about is a tumor -- a brain tumor can mimic vestibular. Vets don't even like to mention it because it can freak you and I out so much. *sigh* (been there, trust me!)
Foxy found a ton of relief when I'd massage his neck (and he had some spondylosis in the lower back too). Try just plain old 'green alcohol' (plain rubbing alcohol with wintergreen in it) .... or you can get peppermint or wintergreen essential oil and try it -- just a couple of drops and massage it in.
Imagine how 'tight' YOUR neck would get if it was 'tilted' all the time -- the muscles on the short side are tense and strained, and the ones on the long side are tired and weak. You don't have to use much oil -- just a drop or two. But it's a great relaxing time for both of you!!
