Challenge #3 -- More than just Billy's ears

    On Saturday we will have a couple of more tests done -- a blood test for CBCs to see if his blood IS responding and another fecal to see if possibly there are any other parasites (like whips, which there shouldn't be because we're using Interceptor).  My vet is trying SO hard -- he's been awesome support.

    I will give you guys the update as soon as I have it.  He went on Saturday for more blood drawn (CBC and packcell volume to see if we are even touching the anemia at all yet) and a fecal (my boy obliged with a nice big sample LOL) to see if it could POSSIBLY be parasite.  They are also going to run an erlichia test on the blood just to see for sure altho he just doesn't fit ANY of the known symptom patterns for any of this. 
     
    This vet we went to on Saturday is not 'my' vet but rather my vet's friend (they started out at the same practice after graduating from Auburn together).  This guy is much more mainstream than my vet -- big old display of SD (Science D_____ fillin the blank as you want to *grin*). 
     
    But when he looked at Billy's gums he said "I can NOT believe this is a dog with a hematacrit of 20 -- he looks in GREAT shape - his gums are pink, he's got lots of energy".  I said quietly ... awww, ok, NOT quietly ... "I homecook -- he gets tons of leafy greens including kale, spinach, etc. and almost no grain". 
     
    His jaw about dropped to the floor.  I'm certainly not what he expected, altho my vet DID prepare him for the arrival of Clan Kennedy.  LOL
     
    Will take that blood a few days to come back.  WE'll see.  Meanwhile I'm trying not to think about it.

     I just got a call from Dr. Bailey. 

    Originally we tested Billy's blood (almost byaccident) on 5/13/06 (Saturday).  We got the low blood "probably IMHA" results on Tuesday 5/16/06 (Tuesday).  THAT NIGHT we started him on the prednisone to suppress the immune system and on Dr. DiNatale's
    immune-stimulating-acts-like-a-decoy-to draw the immune system away from the red blood cells PLUS a blood 'builder' on that same Tuesday.

    Last Saturday 5/20/06 (after only FOUR days of treatment) his blood results are MUCH better.  Not "normal" BUT (trumpets PLEASE!!!)

    *****HE IS RESPONDING TO TREATMENT*****

    See that's a big huge deal ... he's a 'buffy coat cocker' and historically they DO NOT respond to treatment for IMHA (Immune Mediated Hemolytic Anemia) -- they often just plain die.  The fact that the blood values are SO much better SO FAST not only indicates to Dr. B that he's going to 'recover' (and he's never even been  symptomatic!!!) but that he won't have to be on steroids LONG LONG term.

    Now with any sort of immune mediated thing, you literally have to "re-train" the body.  So the pred takes WEEKS and WEEKS and you wean off ULTRA slow.  

    So 3 weeks (total) of 10 mg pred twice a day.  Then to something like 7.5 mg pred twice a day  for probably 3 weeks ... a VERY slow decrease just leading the body down the garden path hopefully so it doesn't even NOTICE it's been snafu'd.  (potentially he could be on pred for months in slightly decreasing doses but we will see about that)

    So now Dr. D (holistic vet) and Dr. B (regular vet) put their heads together and figure out how to handle the herbals. 

    But this is SUCH good news.  In my heart of hearts I was pretty sure it wasn't Erlichia -- it would have been nice to have had some neat little cureable disease ... but he gets Interceptor regularly (which might not be effective even if it is a blood parasite) so I just didn't see how it COULD have been that.  Nor could I see how it could be Whips or Hooks  The only thing Dr. B hasn't seen results on was the fecal -- but we'll see.  (It's been done, they just didn't send him the results.)

    His immune system has always been a mess -- not with demodex but with the allergies.  So I'm not at all surprised at this.  But I want this dog to LIVE a nice long life.  That's ME ... ME ME ME wanting this, to be boldly honest. 

    So thanks folks for the support -- I has meant the world to me.

    IF YOU EVER *THINK* YOUR DOG JUST ISN'T ACTING QUITE 'RIGHT' -- **PURSUE IT**
     
    If you can afford it, blood tests, or get after your vet not to just blow you off with "probably doesn't mean anything".  When it is YOUR dog, often it's YOU who is the only one who will 'see' something.
     
    I'm gonna go do the happy dance AGAIN now.

    Wow Callie - that's awesome news!!  I could hear the trumpets all the way out here in California.  I totally agree about going with your gut when you "think" something just isn't right.  Fortunately, you've got a trustworth gut and you paid attention to it.  Enjoy the happy dance [sm=clapping%20hands%20smiley.gif]

    We just got back from seeing the holistic vet.  She's got a slightly different take from the other vet (not bad, just 'different').
     
    She thinks it was likely that ear infection that actually TRIGGERED the IMHA -- Billy's been prone to skin infections, but this one was potentially nastier in that it was systemic and somehow 'worse'.  So she thinks it's a good possibility that what he's responding to is more the presence of the antibiotic, and she's actually suggesting keeping him on the antibiotic for an EXTRA week.  (yep, this is the holistic vet talking -- the one who hates antibiotics!!)
     
    But she (altho her prognosis is even better than the regular vet's) doesn't think this is a permanent anemia at all.  *I* was under the impression that once you had this, you HAD it forever.  But she says no.
     
    SOMETIMES (in fact, probably 'often' or even 'most of the time') IMHA is triggered by excessive vaccines, steroid use, or drugs (particularly long term antibiotic use of pennicillin type drugs or sulpha drugs like TMZ) you don't find that dog sick until they 'crash' -- constantly vomiting, terribly anemic, weak, lethargic, etc. -- and generally their liver/kidneys have been trashed by long term drug use -- but even THEN often she expects them to get off the pred and return to a more normal state if they have been found in time TO help them. 
     
    Of course, like me, she wants to see him off pred as soon as possible -- not 'fast' -- she agrees totally that we have to decrease the pred by small increments -- far smaller than you would normally wean off pred.  But not months and months either.
     
    So now I straddle the regular vet/holistic vet 'line' and pretty well go with my gut.  She and I talked a LOT about food.  She wants super nourishing stuff.  Barley (particularly I need to find Oriental Barley which is 'different' somehow) and greens (which he already gets a LOT of and she was thinking that's part of why he HAS fared so well).  She'd like to see me do a lot of iron rich meats -- he already gets beef, but she wants heart/liver, etc.
     
    And LOTS and LOTS and LOTS of milk thistle to cleanse the liver from the pred he's taking in.
     
    But not much that is detoxing or cleansing YET -- until he heals first.
     
    I feel SO much better -- I was really a bit freaked that this was a 'forever' thing that would overshadow us into oblivion.  But I guess the fact that we caught it EARLY makes such an enormous difference. 
     
    Oh ... and the other thing she suggested ... since we're going to cautiously wean him off the pred (and aren't even sure when he's totally returned to 'normal' BEFORE we cut the pred back) she suggested I bring him in once a week on the evening she's open late (and I can't GET to my other vet like this), but that they can pull just a small sampel to get a hematacrit in just a couple of minutes and literally check the blood weekly.  That way if we drop the pred and the blood drops suddenly we'll know we're in trouble RIGHT away.
     
    We're definitely gonna do that.
     
    There is only one more test to come back and that's the fecal ... so parasites aren't 100% ruled out.  Poor Billy -- he just ain't following ANY known 'parameter'.  But at least we're making progress.  So that's SOMETHIN!!!
     
    Thanks again -- and this is the boy with his 'long' hair.  During the winter we usually let it grow because David and I both like him soft and silky.  This was a cold day at the park and his Scotland the Brave shirt (Dad's favorite).  And English cocker spanYELL, in a Scottish shirt -- now THAT is *brave*!!! LOL

    I just wanted to also say I couldn't agree more with you, if you think something isn't right encourage the vet to look into it.
     
    The last time Willow went in after not eating for awhile, I had sent a note with DH for them to take blood work.  Good thing I did, that's when we discovered the low white blood cell count.  If I hadn't asked, they would of sent her home saying she was fine and that it was behavioral.  I know it's not ALL behavioral.  I also asked them to weigh her, she had lost weight it turned out.  I asked them to check her temp, it was slightly high. 
     
    I know they would of just dismissed me if I hadn't asked specifically.  They would of thought I was just a worry wart and spoiling her.
