You need to keep him from licking off that lotrimin -- are you using cream or liquid? Are you bathing him? Ringworm will spread and it IS contagious to you. I'm a bit surprised the vet didn't give you something like Malaseb to bathe the dog in -- you have to leave it on for 10-15 minutes (and yeah, you might actually have to put a gauze muzzle on him to keep him from licking).
I would likely be using a back up of either lavender or tea tree oils - both are decent anti-fungals. Just the tiniest bit massaged in will help back this up.
My guess is that the oral drug is far less effective (and it is INCREDIBLY hard on the liver - please please please give this dog milk thistle at least twice daily -- the adult human dose - just to help support and detox the body). And the dog is licking off the topical.
So honestly ... altho this is difficult, even if you have to sit with this dog and hold the muzzle shut with your hand to give the medicine time to penetrate and do some good, you need to go the extra mile here to make it be effective. It's important if for no other reason than it is contagious to you as well.
