Seeking help for a story on anal area problems

I'm a journalist who writes regularly about medical issues for Your Dog, the newsletter for Cummings School of Veterinary Medicine at Tufts University.

For all of my stories, I not only interview medical experts but also look for owners of dogs that have the conditions and have gone for treatment. I'm interested in knowing how they discovered the problem, what treatments were tried (medical, surgical, dietary), and how successful they were.

So... I am currently doing a story on different diseases of the anal area -- not a topic people necessarily want to talk about in polite company but very common in many breeds and often difficult to deal with. Specifically I'm looking for:

    • anal sac problems: Dogs whose sacs were impacted regularly, particularly common in small dogs
    • perianeal fistulas: particularly common in German shepherds but also occurring in Irish setters and some Alpine breeds
    • perinanal hernias: particularly common in male dogs that have not been neutered

The stories usually run pictures of any of the subjects of the interviews (and I promise, the pictures would be of your dog at his or her best; no butt photos)!

I'm new to this forum so I'm not sure where to go from here, but would like to conduct phone interviews with anyone willing to help. I wouldn't ask anyone to post a phone number here, but we can figure out the logistics on the forum.

Thanks in advance for any help you can give me!

I have a 5 year old german shepherd who has been dealing with perianal fistulas for close to a year. I would be happy to answer any questions you have.

As far as how i discovered it- i noticed he was licking the area a whole lot, and when i lifted up his tail, the anus was reddened and swollen. I was told it was a ruptured anal sac and given antibiotics. A few days later he was acting extremely ill, and i took him to a different vet. He did have a rectal obstruction that was making him so sick, but they diagnosed the PF at that time.

Treatments- first regimen was cyclosporine, ketoconazole, cephalexin, flagyl, reglan, and metacam. This worked for awhile, but after treating with these for about 6 months, the fistulas were back and he was worse. We have now been on topical tacrolimus with rounds of prednisone thrown in. He has not been doing very well. We talked about trying pred+immuran.

Since he's had the diagnosis, he has lost 18 lbs and has to wear a diaper.

Let me know anything else i can tell you, i'd be happy to share.

Thanks so much for writing. As I mentioned in my private email to you, Megan, I'm not sure how to handle a story of a problem that's not been successfully resolved but it occurred to me to put it out here: Has anyone had a similar experience to Megan's and found something that resolved the problem?
