I asked my vet last week about a round spot on my dog's leg, also. Only, his doesn't have that sore, irritable area in the center. His is just lost hair. The hair around the area pulls out easily. She said it might be ringworm, as luv4gsds said. Or it could be more or less, just a sore.
She suggested putting neosporin with pain relief on there (which, honesly, I haven't done since it hasn't gotten worse). She said the pain relief has a numbing agent that will make dog's tonques numb and they won't like it and should leave it alone.
I *think* more round spots will show up if it is ringworm, but maybe not? Ringworm is a fungal infection and can be transmitted to humans, if that's what it is.
I think the center area really looks nasty, as far as irritations go. The picture seems to indicate red bumpy areas. Not having experience with this, I can't say what it might be, but that concerns me more than just the hair loss in one area.
If it were me, I would have it looked at. If it's ringworm, then you need meds. If not, then I would like to know why there are multiiply red bumps in the area. Since you say it doesn't seem to bother her, then that makes me think it is not her biting the wound during the day. I think if it "itched or hurt", then you would see her biting at night. Since it doesn't appear to bother her, then I think there is less chance she is "bothered' by it during the day. While she could be "picking" on it out of boredom during the day, I honestly don't think that is likely. I would have it checked out.
