I'm sure if this was an option, your vet would've suggested it ---- but if he can't keep pain meds down, could he have a pain patch or an injection rather than having to swallow pills that aren't getting digested quickly enough?
It's a Catch-22 . . . he needs the pain meds to feel better, but they contribute to his nausea, which makes him feel worse, so he won't eat a lot, and if he's vomiting them undigested, they're not helping the pain, so his appetite continues to lessen (I'm sure you already have this circular thinking -- but that's where my head keeps going!).
I just keep thinking what would happen with a human patient in a similar situation. It seems like there would be IV's for nutrition and pain meds, right? Or do human patients have to continually try to swallow pills and eat food when recovering from a critical injury? Thankfully, I've not had first-hand experience with such things, so I just don't know. And I realize it's probably much different for animal care procedures. I'm glad your vet gives such good guidance. It helps a lot to be able to say: "What would you do if he were your beloved dog?" and know he'll give you an honest, measured, compassionate answer.
