    Biceps Tenosynovitis (Bugsy)

Bugsy started to have a front end limp in August 2012 (actually it first appeared in June 2011 but went away).

We had an accurate diagnosis of biceps tenosynovitis (a tendonitis/bursitis at the point of insertion of the biceps tendon) in September and were told then that there are limited options for treatment and that in all likelihood it would be something that he dealt with on and off for the rest of his life. As you can imagine I found that difficult to accept because it severely limits his activity and he remains an active dog despite being 'old' for his size dog.

But we did as asked and removed any formal walks and only swam for exercise. He was doing well through October swimming several times a week and building up to some long distance swimming. However, he still couldn't walk without a limp.

Then due to a variety of reasons we couldn't swim and the limp came back furiously. This was November.

More complete rest but no real improvement. Some days not too bad so we did some exercise other days it was horrific.

I tried and tried to convince the vets that if he was limping that badly it was REALLY bad and kept asking about whether it was a torn tendon at this point. But they just kept saying that this tendonitis/bursitis is very painful and that if it was torn he couldn't use it. I really did my best to get them to believe that he would always try to use it to no avail.

We've been to the ortho vet 3x and the regular vet 3x. The last visit to the ortho vet I was told to try massage and acupuncture.

We tried massage and that was an epic fail due to Bugsy's nature

We set up an acupuncture appointment for Thursday 2/14/13 and we added some things to his diet from Chinese medicine

On Saturday he decided to run zoomies in the yard. He just has to run like that daily, always has and it is a wonderful expression of who he is.

Unfortunately he tried one figure eight too many and became 3 legged. He just stood there on three legs with a happy look on his face and his tail wagging. When he walked 3 legged I knew in my heart that he'd blown it out completely.

We've been doing tramadol and rimadyl 24/7 and he is still basically 3 legged. He toe touches and uses it to go down stairs but that is about it.

I had read way back in September that there is a surgery in which they cut the tendon and the dog heals. I was told it was drastic blah blah blah. Well now that I've wasted 6 months of Bugsy's life doing basically nothing and not having any improvement followed by this event I am going to find someone to do the surgery. The surgery is arthroscopic which isn't all too common yet in veterinary care but I have located several places all within a 6 hr drive of us.

Here are some links to the surgery, should you want to know more:

It sure sounds like he's torn it completely and the arthro surgery is looking like the best option.  Good luck and speedy appointment vibes.

Poor Bugsy.  Hopefully he can get some relief soon.

Karen -- you are SO "Dog Mom of the Year" -- talk about being an advocate for YOUR dog.  You understand him -- you get in his head and truly "get" what makes this dog that pure element called "Bugsy".  You haven't just taken the pat answer -- and now you're willing to go the extra mile (literally) for him.

I hope you find the perfect vet to do this for you.

Wow -- what an ordeal.  Like Callie said, it's awesome that you've been doing everything you can to get to the bottom of things, seeing so many vets and trying everything possible to help.  And while no one wants to see their dog go through a surgery, if it ultimately brings relief, then it's worth it.  How fortunate that you have options that aren't insanely far away and that you have a work schedule that will allow you to make the trip wherever you need to go!

As someone with a dog who has to zoom every single day (and who cringes each time I see her slip or skid in the wet yard), I understand how impossible it is to suppress that bursting level of energy dogs like Bugsy and Ruby have.  I expect we'll have some kind of injuries or surgeries in our future with Ruby, so you'll be one of the people I look to for info and opinions!

We'll be anxious to hear what the plan is this week!

I'm really, really hopeful that you are finally going to be able to get the treatment you've known he needs. I'll be thinking about you both in the days ahead.

well its been a day of phone calls and research and confusion. What I thought was settled at 10 am is no longer settled BUT my neighbor the vet that works at the vet school is going to walk with me on Wednesday morning and help me sort through all of this.

meanwhile Bugsy is happy and playful as per usual :)

     Thanks Janice (and everyone)

It has truly been a mind-boggling day in many ways - the same surgical procedure can be had for $800 or for $3800. Some refuse to do anything without an MRI (another $2000) others are ready to go.

It has become clear that there could be a different tendon in play (although I was just reading about that and feel 98% sure its the biceps tendon) and that if I went ahead with the arthroscopy on the biceps tendon they might just find it fine. Now I find that a VERY unlikely scenario but its good to know it could be the issue.

At this moment - prior to talking to my friend - I am leaning toward getting a cortisone injection in the biceps tendon and if that relieves the pain we know that is the tendon. At that point I would go ahead with getting it cut.

I don't really want to wait any longer to 'fix' it but I need to feel 100% on my decision.

I will speak to my usual vet tomorrow and as I said my friend on Wednesday and go from there. I can guarantee that I will be doing something proactive very soon.

Just as a little smile, Bugsy is thrilled that the vet that does acupuncture gave me some things to add to his diet to help with his 'energy' and well being. One of the things is liver and doesn't matter whose liver or whether its frozen or room temp or cooked he is darn happy with daily liver. I have also discovered that he loves fresh ginger Surprise

Definitely Dog Mom of the Year!!!! Hope everything is OK and you are doing awesome by Bugsy!!

You certainly are dog mom of the year. So you may have said this already but has he been into the vet yet? It sounds like you aren't 100%sure what it is. Wouldn't you need the mri to make sure of what type of procedure is needed? You have insurance right? Will they require a vet diagnosis first? So many questions!

I think you're on the right track.  Why spend that money for an MRI if the steroid shot can pinpoint the tendon involved?  I know how frustrated you are with the whole situation but I know you'll make the right decision once you have as much info as you can gather.  Sending you lots of calming vibes as you work this out.

Liver and fresh ginger?  Yeah, who wouldn't love that? LOL
