What is this on his ears?

(scraped for sarcops and it absolutely could *not* be sarcops when the dog isn't itching but it's good for an extra few bucks on the bill).  It tends to be one of those experiential things that a vet knows when they see it (and a new vet out of vet school probably hasn't seen it so guesses a lot).
