    Evans Syndrome

    My 6 yr. old female Alapaha Bull Dog Choco is dealing with a relapse of her Evans. Last time she was brought in with a PCV of 19 and platelet count below 10,000. Through a lot of fight and two transfusions she was in remission for a year and being weaned off of the drugs. (Atopica, Pred, Mychophenalate, & prilosec. She was drug free for a year until Saturday night I was uneasy about her vomiting bile for two days. I got her in to the vet who just said to take Prilosec. On Sunday something was off. She still had pink gums and good color but it did not feel right so I rand my next door neighbors who are both vets and they said to bring her over. In 60 seconds they identified an enlarged spleen and some other things and said to get her to the hospital. (we have a great one by us) Her pcv was 42 but her platelets were 10,000. She was started up on all the meds again and a new one that is a chemo drug that has a nice side effect of making the body produce platelets. On Tuesday her pcv dropped to 32 and her platelets moved up slightly to 13,000. They checked PCV again last night (Tuesday) and it dropped to 29. Platelets getting checked again in the next hour as well as pcv. Having been through this before I am trying not to read too much in to the numbers. (treat the patient not the numbers) She is happy, pink, and eating great. I am wondering if I could look at the smaller drop as a positive? It did drop but not even close to as much. Her spleen is back to normal size as well. Any past experiences that could shed some light on my bully's current situation? Thanks -rts

    Rob -- there is another thread on here "AIHA or IMHA" that will be helpful for you (IMT is platelets, IMHA is red blood, and Evan's is both -- YOU know that but I just want to clarify for anyone reading this).

    My email is callieatcritturs@yahoo.com -- feel free to email me. I can send you some info.  An enlarged spleen can be several things -- and the fact that the size has gone down likely says there was inflammation -- did the vet do enough bloodwork to know how high the white cells were?

    Can you tell me where abouts you are located?  (I don't want to walk to your house -- just know your weather, what may be going on 'outside', etc which can all figure into triggers)

    The slowing down of the destruction is always a darned good sign.

    If the vet hasn't done a **complete** chemistry (bloodwork) ask for it -- all the liver, kidney and all the other enzymes, values -- because that can also shed light on where inflammation may originate from.  It will also tell **YOU** where the liver values are (if they are high then the liver has a hard time dealing with the drugs).  If you want to email me I can send you more information.  My Billy had IMHA 9 years ago -- he had six transfusions in a month -- trust me, I get it and I'm still working with folks with IMHA and Evan's dogs.
